Original commit message:

  tcp: support customizing TCP_KEEPINTVL and TCP_KEEPCNT

  Implement `uv_tcp_keepalive_ex` function that extends
  `uv_tcp_keepalive` to support `TCP_KEEPINTVL` and `TCP_KEEPCN`
  socket options in addition to TCP_KEEPIDLE.
