Merge reason: Resolve the conflict

Note, i picked the version from Linus's tree, which effectively reverts
the fs-writeback.c bits of:

  b97181f: fs: remove all rcu head initializations, except on_stack initializations

As the upstream changes to this file changed this code heavily and the
first attempt to resolve the conflict resulted in a non-booting kernel.
It's safer to re-try this portion of the commit cleanly.

On some configs the following build error triggers:

Because asm/apic.h was only included implicitly. Include it explicitly.

Ingo Molnar e3174cfd2a Revert "perf: Fix exit() vs PERF_FORMAT_GROUP"
This reverts commit 4fd38e4595.

It causes various crashes and hangs when events are activated.

The cause is not fully understood yet but we need to revert it
because the effects are severe.

Ingo Molnar 56f0e74c9c x86: Fix parse_reservetop() build failure on certain configs
Commit e67a807 ("x86: Fix 'reservetop=' functionality") added a
fixup_early_ioremap() call to parse_reservetop() and declared it
in io.h.

But asm/io.h was only included indirectly - and on some configs
not at all, causing a build failure on those configs.

Ingo Molnar e58aa3d2d0 genirq: Run irq handlers with interrupts disabled
Running interrupt handlers with interrupts enabled can cause stack
overflows. That has been observed with multiqueue NICs delivering all
their interrupts to a single core. We might band aid that somehow by
checking the interrupt stacks, but the real safe fix is to run the irq
handlers with interrupts disabled.

Drivers for whacky hardware still can reenable them in the handler
itself, if the need arises. (They do already due to lockdep)

The risk of doing this is rather low:

 - lockdep already enforces this
 - CONFIG_NOHZ has shaken out the drivers which relied on jiffies updates
 - time keeping is not longer sensitive to the timer interrupt being delayed

Ingo Molnar 55632770d7 perf events: Fix false positive build warning with older GCC's
gcc 4.2.1 produces:

 util/probe-event.c: In function 'add_perf_probe_events':
 util/probe-event.c:883: warning: 'tev' may be used uninitialized in this function
 make: *** [util/probe-event.o] Error 1

Newer GCCs get this right.

To work it around, initialize the variable to NULL so that older GCCs see
it as initialized too.

Ingo Molnar 3b0d516463 perf probe: Fix !dwarf build
Fix the !drawf build.

This uses the existing NO_DWARF_SUPPORT mechanism we use for that,
but it's really fragile and needs a cleanup. (in a separate patch)

1) Such uses:

 #ifndef NO_DWARF_SUPPORT

are double inverted logic a'la 'not not'. Instead the flag should
be called DWARF_SUPPORT.

2) Furthermore, assymetric #ifdef polluted code flow like:

        if (need_dwarf)
 #ifdef NO_DWARF_SUPPORT
                die("Debuginfo-analysis is not supported");
 #else   /* !NO_DWARF_SUPPORT */
                pr_debug("Some probes require debuginfo.\n");

        fd = open_vmlinux();

is very fragile and not acceptable. Instead of that helper functions
should be created and the dwarf/no-dwarf logic should be separated more
cleanly.

3) Local variable #ifdefs like this:

 #ifndef NO_DWARF_SUPPORT
        int fd;
 #endif

Are fragile as well and should be eliminated. Helper functions achieve
that too.

Ingo Molnar 2aa2b50dd6 x86/mce: Fix build bug with CONFIG_PROVE_LOCKING=y && CONFIG_X86_MCE_INTEL=y
Commit f56e8a076 "x86/mce: Fix RCU lockdep splats" introduced the
following build bug:

  arch/x86/kernel/cpu/mcheck/mce.c: In function 'mce_log':
  arch/x86/kernel/cpu/mcheck/mce.c:166: error: 'mce_read_mutex' undeclared (first use in this function)
  arch/x86/kernel/cpu/mcheck/mce.c:166: error: (Each undeclared identifier is reported only once
  arch/x86/kernel/cpu/mcheck/mce.c:166: error: for each function it appears in.)

Move the in-the-middle-of-file lock variable up to the variable
definition section, the top of the .c file.

Ingo Molnar ba7e4d13fc perf, x86: Add INSTRUCTION_DECODER config flag
The PEBS+LBR decoding magic needs the insn_get_length() infrastructure
to be able to decode x86 instruction length.

So split it out of KPROBES dependency and make it enabled when either
KPROBES or PERF_EVENTS is enabled.

Ingo Molnar 94d8f785dd dell-laptop: Fix build error by making buffer_mutex static
The following build bug (x86, allyesconfig):

  arch/x86/oprofile/built-in.o:(.data+0x250): multiple definition of `buffer_mutex'

Was triggered in -tip testing, caused by this upstream commit:

  116ee77: dell-laptop: Use buffer with 32-bit physical address

There's multiple buffer_mutex's in the kernel. Make this new one
static.

Ingo Molnar 21c2fd9970 x86: apic: Fix mismerge, add arch_probe_nr_irqs() again
Merge commit aef55d4922 mis-merged io_apic.c so we lost the
arch_probe_nr_irqs() method.

This caused subtle boot breakages (udev confusion likely
due to missing drivers) with certain configs.
