David Howells ff8e210a95 AFS: fix file locking
Fix file locking for AFS:

 (*) Start the lock manager thread under a mutex to avoid a race.

 (*) Made the locking non-fair: New readlocks will jump pending writelocks if
     there's a readlock currently granted on a file. This makes the behaviour
     similar to Linux's VFS locking.

David Howells 0b1937ac0e FRV: Fix linkage problems
Make it possible to use __start_notes and __stop_notes without getting a GPREL
overflow error from the FRV linker.

Small variables that would otherwise be in .data or .bss may, depending on the
arch, be placed in special sections (.sdata or .sbss) that permit single
instruction references on fixed instruction width machines.

__start_notes and __stop_notes aren't really char variables, and certainly
don't refer to data in .data or .bss. Making them type "void" fools the
compiler into not assuming anything about them.

Fix afs_send_simple_reply() to accept a greater-than-zero return value from

successfully, but has been patched to return the number of bytes it

David Howells ea02e3dde3 FRV: work around a possible compiler bug
Work around a possible bug in the FRV compiler.

What appears to be happening is that gcc resolves the
__builtin_constant_p() in kmalloc() to true, but then fails to reduce the
therefore constant conditions in the if-statements it guards to constant
results.

When compiling with -O2 or -Os, one single spurious error crops up in
cpuup_callback() in mm/slab.c. This can be avoided by making the memsize
variable const.

David Howells 0f300ca928 AFS: fix a couple of problems with unlinking AFS files
Fix a couple of problems with unlinking AFS files.

 (1) The parent directory wasn't being updated properly between unlink() and

     It seems that, for some reason, invalidate_remote_inode() wasn't
     discarding the directory contents correctly, so this patch calls

David Howells 5bbf5d39f8 AFS: further write support fixes
Further fixes for AFS write support:

 (1) The afs_send_pages() outer loop must do an extra iteration if it ends
     with 'first == last' because 'last' is inclusive in the page set

     some circumstances.

 (2) Similarly, the outer loop in afs_pages_written_back() must also do an

It added a "mark_page_accessed(page)" at the final return path in

David Howells 31143d5d51 AFS: implement basic file write support
Implement support for writing to regular AFS files, including:

 (1) write

 (2) truncate

 (3) fsync, fdatasync

 (4) chmod, chown, chgrp, utime.

AFS writeback attempts to batch writes into as chunks as large as it can manage
up to the point that it writes back 65535 pages in one chunk or it meets a
locked page.

Furthermore, if a page has been written to using a particular key, then should
another write to that page use some other key, the first write will be flushed
before the second is allowed to take place. If the first write fails due to a
security error, then the page will be scrapped and reread before the second
write takes place.

If a page is dirty and the callback on it is broken by the server, then the
dirty data is not discarded (same behaviour as NFS).

Shared-writable mappings are not supported by this patch.

David Howells 224711df5c [AF_RXRPC]: Sort out MTU handling.
Sort out the MTU determination and handling in AF_RXRPC:

 (1) If it's present, parse the additional information supplied by the peer at
     the end of the ACK packet (struct ackinfo) to determine the MTU sizes
     that peer is willing to support.

 (2) Initialise the MTU size to that peer from the kernel's routing records.

 (3) Send ACKs rather than ACKALLs as the former carry the additional info,
     and the latter do not.

 (4) Declare the interface MTU size in outgoing ACKs as a maximum amount of
     data that can be stuffed into an RxRPC packet without it having to be
     fragmented to come in this computer's NIC.

 (5) If sendmsg() is given MSG_MORE then it should allocate an skb of the
     maximum size rather than one just big enough for the data it's got left
     to process on the theory that there is more data to come that it can
     append to that packet.

     This means, for example, that if AFS does a large StoreData op, all the
     packets barring the last will be filled to the maximum unfragmented size.

David Howells 651350d10f [AF_RXRPC]: Add an interface to the AF_RXRPC module for the AFS filesystem to use
Add an interface to the AF_RXRPC module so that the AFS filesystem module can
more easily make use of the services available. AFS still opens a socket but
then uses the action functions in lieu of sendmsg() and registers an intercept
functions to grab messages before they're queued on the socket Rx queue.

This permits AFS (or whatever) to:

 (1) Avoid the overhead of using the recvmsg() call.

 (2) Use different keys directly on individual client calls on one socket
     rather than having to open a whole slew of sockets, one for each key it
     might want to use.

 (3) Avoid calling request_key() at the point of issue of a call or opening of
     a socket. This is done instead by AFS at the point of open(), unlink() or
     other VFS operation and the key handed through.

 (4) Request the use of something other than GFP_KERNEL to allocate memory.

Furthermore:

 (*) The socket buffer markings used by RxRPC are made available for AFS so
     that it can interpret the cooked RxRPC messages itself.

 (*) rxgen (un)marshalling abort codes are made available.

The following documentation for the kernel interface is added to
Documentation/networking/rxrpc.txt:

=========================
AF_RXRPC KERNEL INTERFACE
=========================

The AF_RXRPC module also provides an interface for use by in-kernel utilities
such as the AFS filesystem. This permits such a utility to:

 (1) Use different keys directly on individual client calls on one socket
     rather than having to open a whole slew of sockets, one for each key it
     might want to use.

 (2) Avoid having RxRPC call request_key() at the point of issue of a call or
     opening of a socket. Instead the utility is responsible for requesting a
     key at the appropriate point. AFS, for instance, would do this during VFS
     operations such as open() or unlink(). The key is then handed through
     when the call is initiated.

 (3) Request the use of something other than GFP_KERNEL to allocate memory.

 (4) Avoid the overhead of using the recvmsg() call. RxRPC messages can be
     intercepted before they get put into the socket Rx queue and the socket
     buffers manipulated directly.

To use the RxRPC facility, a kernel utility must still open an AF_RXRPC socket,
bind an addess as appropriate and listen if it's to be a server socket, but
then it passes this to the kernel interface functions.

The kernel interface functions are as follows:

 (*) Begin a new client call.

	struct rxrpc_call *
	rxrpc_kernel_begin_call(struct socket *sock,
				struct sockaddr_rxrpc *srx,
				struct key *key,
				unsigned long user_call_ID,
				gfp_t gfp);

     This allocates the infrastructure to make a new RxRPC call and assigns
     call and connection numbers. The call will be made on the UDP port that
     the socket is bound to. The call will go to the destination address of a
     connected client socket unless an alternative is supplied (srx is
     non-NULL).

     If a key is supplied then this will be used to secure the call instead of
     the key bound to the socket with the RXRPC_SECURITY_KEY sockopt. Calls
     secured in this way will still share connections if at all possible.

     The user_call_ID is equivalent to that supplied to sendmsg() in the
     control data buffer. It is entirely feasible to use this to point to a
     kernel data structure.

     If this function is successful, an opaque reference to the RxRPC call is
     returned. The caller now holds a reference on this and it must be
     properly ended.

 (*) End a client call.

	void rxrpc_kernel_end_call(struct rxrpc_call *call);

     This is used to end a previously begun call. The user_call_ID is expunged
     from AF_RXRPC's knowledge and will not be seen again in association with
     the specified call.

 (*) Send data through a call.

	int rxrpc_kernel_send_data(struct rxrpc_call *call, struct msghdr *msg,
				   size_t len);

     This is used to supply either the request part of a client call or the
     reply part of a server call. msg.msg_iovlen and msg.msg_iov specify the
     data buffers to be used. msg_iov may not be NULL and must point
     exclusively to in-kernel virtual addresses. msg.msg_flags may be given
     MSG_MORE if there will be subsequent data sends for this call.

     The msg must not specify a destination address, control data or any flags
     other than MSG_MORE. len is the total amount of data to transmit.

 (*) Abort a call.

	void rxrpc_kernel_abort_call(struct rxrpc_call *call, u32 abort_code);

     This is used to abort a call if it's still in an abortable state. The
     abort code specified will be placed in the ABORT message sent.

 (*) Intercept received RxRPC messages.

	typedef void (*rxrpc_interceptor_t)(struct sock *sk,
					    unsigned long user_call_ID,
					    struct sk_buff *skb);

	void
	rxrpc_kernel_intercept_rx_messages(struct socket *sock,
					   rxrpc_interceptor_t interceptor);

     This installs an interceptor function on the specified AF_RXRPC socket.
     All messages that would otherwise wind up in the socket's Rx queue are
     then diverted to this function. Note that care must be taken to process
     the messages in the right order to maintain DATA message sequentiality.

     The interceptor function itself is provided with the address of the socket
     and handling the incoming message, the ID assigned by the kernel utility
     to the call and the socket buffer containing the message.

     The skb->mark field indicates the type of message:

	MARK				MEANING
	===============================	=======================================
	RXRPC_SKB_MARK_DATA		Data message
	RXRPC_SKB_MARK_FINAL_ACK	Final ACK received for an incoming call
	RXRPC_SKB_MARK_BUSY Client call rejected as server busy
	RXRPC_SKB_MARK_REMOTE_ABORT	Call aborted by peer
	RXRPC_SKB_MARK_NET_ERROR	Network error detected
	RXRPC_SKB_MARK_LOCAL_ERROR	Local error encountered
	RXRPC_SKB_MARK_NEW_CALL New incoming call awaiting acceptance

     The remote abort message can be probed with rxrpc_kernel_get_abort_code().
     The two error messages can be probed with rxrpc_kernel_get_error_number().
     A new call can be accepted with rxrpc_kernel_accept_call().

     Data messages can have their contents extracted with the usual bunch of
     socket buffer manipulation functions. A data message can be determined to
     be the last one in a sequence with rxrpc_kernel_is_data_last(). When a
     data message has been used up, rxrpc_kernel_data_delivered() should be
     called on it..

     Non-data messages should be handled to rxrpc_kernel_free_skb() to dispose
     of. It is possible to get extra refs on all types of message for later
     freeing, but this may pin the state of a call until the message is finally
     freed.

 (*) Accept an incoming call.

	struct rxrpc_call *
	rxrpc_kernel_accept_call(struct socket *sock,
				 unsigned long user_call_ID);

     This is used to accept an incoming call and to assign it a call ID. This
     function is similar to rxrpc_kernel_begin_call() and calls accepted must
     be ended in the same way.

     If this function is successful, an opaque reference to the RxRPC call is
     returned. The caller now holds a reference on this and it must be
     properly ended.

 (*) Reject an incoming call.

	int rxrpc_kernel_reject_call(struct socket *sock);

     This is used to reject the first incoming call on the socket's queue with
     a BUSY message. -ENODATA is returned if there were no incoming calls.
     Other errors may be returned if the call had been aborted (-ECONNABORTED)
     or had timed out (-ETIME).

 (*) Record the delivery of a data message and free it.

	void rxrpc_kernel_data_delivered(struct sk_buff *skb);

     This is used to record a data message as having been delivered and to
     update the ACK state for the call. The socket buffer will be freed.

 (*) Free a message.

	void rxrpc_kernel_free_skb(struct sk_buff *skb);

     This is used to free a non-DATA socket buffer intercepted from an AF_RXRPC
     socket.

 (*) Determine if a data message is the last one on a call.

	bool rxrpc_kernel_is_data_last(struct sk_buff *skb);

     This is used to determine if a socket buffer holds the last data message
     to be received for a call (true will be returned if it does, false
     if not).

     The data message will be part of the reply on a client call and the
     request on an incoming call. In the latter case there will be more
     messages, but in the former case there will not.

 (*) Get the abort code from an abort message.

	u32 rxrpc_kernel_get_abort_code(struct sk_buff *skb);

     This is used to extract the abort code from a remote abort message.

 (*) Get the error number from a local or network error message.

	int rxrpc_kernel_get_error_number(struct sk_buff *skb);

     This is used to extract the error number from a message indicating either
     a local error occurred or a network error occurred.

David Howells aa289b4723 [PATCH] FDPIC: fix the /proc/pid/stat representation of executable boundaries
Fix the /proc/pid/stat representation of executable boundaries. It should
show the bounds of the executable, but instead shows the bounds of the
loader.

Before the patch is applied, the bug can be seen by examining, say, inetd:

	# ps | grep inetd
	  610         root          0   S   /usr/sbin/inetd -i
	# cat /proc/610/maps

 	3273467584 0 0 4096 90115 3221712796 0 0 17 0 0 0 0

The code boundaries are 3233480704 to 3233523592, which are:

	(gdb) p/x 3233480704
	$1 = 0xc0bb0000
	(gdb) p/x 3233523592
	$2 = 0xc0bba788

Which corresponds to this line in the maps file:

	c0bb0000-c0bba788 r-xs 00000000 00:0b 14582157  /lib/ld-uClibc-0.9.28.so

Which is wrong. After the patch is applied, the maps file is pretty much
identical (there's some minor shuffling of the location of some of the
anonymous VMAs), but the stat file is now:

	# cat /proc/610/stat
	610 (inetd) S 1 610 610 0 -1 256 0 0 0 0 0 7 0 0 18 0 1 0 94392000722

	3273467584 0 0 4096 90115 3221712796 0 0 17 0 0 0 0

The code boundaries are then 3276111872 to 3276141668, which are:

	(gdb) p/x 3276111872
	$1 = 0xc3458000
	(gdb) p/x 3276141668
	$2 = 0xc345f464

And these correspond to this line in the maps file instead:

	c3458000-c345f464 r-xs 00000000 00:0b 16384612  /usr/sbin/inetd

Which is now correct.

David Howells 6bdd61d876 IB/mthca: Work around gcc bug on sparc64
For some reason gcc-3.4.5 on sparc64 does:

 WARNING: "____ilog2_NaN" [drivers/infiniband/hw/mthca/ib_mthca.ko] undefined!

Points to note:

 (1) The asm volatile flush/flushw are just markers for viewing what comes out
     in the assembly; removing them has no effect on the result.

 (2) Changing almost anything else in dwh__mthca_arbel_init_srq_context() or
     dwh__mthca_alloc_srq() causes the problem to go away.

The compiler command line issued by the kernel build is:

/opt/crosstool/gcc-3.4.5-glibc-2.3.6/sparc64-unknown-linux-gnu/bin/sparc64-unknown-linux-gnu-gcc -fno-strict-aliasing -fno-common -Os -m64 -mno-fpu -mcpu=ultrasparc -mcmodel=medlow -ffixed-g4 -ffixed-g5 -fcall-used-g7 -Wa,--undeclared-regs -pg -fno-omit-frame-pointer -fno-optimize-sibling-calls -fasynchronous-unwind-tables -g  -c -o drivers/infiniband/hw/mthca/.tmp_mthca_srq.o drivers/infiniband/hw/mthca/mthca_srq.c

This can be reduced to this whilst still retaining the problem:

David Howells 9ad0830f30 [PATCH] Keys: Fix key serial number collision handling
Fix the key serial number collision avoidance code in key_alloc_serial().

This didn't use to be so much of a problem as the key serial numbers were
allocated from a simple incremental counter, and it would have to go through
two billion keys before it could possibly encounter a collision. However, now
that random numbers are used instead, collisions are much more likely.

This is fixed by finding a hole in the rbtree where the next unused serial
number ought to be and using that by going almost back to the top of the
insertion routine and redoing the insertion with the new serial number rather
than trying to be clever and attempting to work out the insertion point
pointer directly.

This fixes kernel BZ #7727.

David Howells 4594bf159f [PATCH] WorkStruct: Use direct assignment rather than cmpxchg()
Use direct assignment rather than cmpxchg() as the latter is unavailable
and unimplementable on some platforms and is actually unnecessary.

The use of cmpxchg() was to guard against two possibilities, neither of
which can actually occur:

 (1) The pending flag may have been unset or may be cleared. However, given
     where it's called, the pending flag is _always_ set. I don't think it
     can be unset whilst we're in set_wq_data().

     Once the work is enqueued to be actually run, the only way off the queue
     is for it to be actually run.

     If it's a delayed work item, then the bit can't be cleared by the timer
     because we haven't started the timer yet. Also, the pending bit can't be
     cleared by cancelling the delayed work _until_ the work item has had its
     timer started.

 (2) The workqueue pointer might change. This can only happen in two cases:

     (a) The work item has just been queued to actually run, and so we're
         protected by the appropriate workqueue spinlock.

     (b) A delayed work item is being queued, and so the timer hasn't been
     	 started yet, and so no one else knows about the work item or can
     	 access it (the pending bit protects us).

     Besides, set_wq_data() _sets_ the workqueue pointer unconditionally, so
     it can be assigned instead.

So, replacing the set_wq_data() with a straight assignment would be okay
in most cases.

The problem is where we end up tangling with test_and_set_bit() emulated
using spinlocks, and even then it's not a problem _provided_
test_and_set_bit() doesn't attempt to modify the word if the bit was
set.

If that's a problem, then a bitops-proofed assignment will be required -
equivalent to atomic_set() vs other atomic_xxx() ops.

David Howells f0d1b0b30d [PATCH] LOG2: Implement a general integer log2 facility in the kernel
This facility provides three entry points:

	ilog2() Log base 2 of unsigned long
	ilog2_u32()	Log base 2 of u32
	ilog2_u64()	Log base 2 of u64

These facilities can either be used inside functions on dynamic data:

	int do_something(long q)
	{
		...;
		y = ilog2(x)
		...;
	}

Or can be used to statically initialise global variables with constant values:

	unsigned n = ilog2(27);

When performing static initialisation, the compiler will report "error:
initializer element is not constant" if asked to take a log of zero or of
something not reducible to a constant. They treat negative numbers as
unsigned.

When not dealing with a constant, they fall back to using fls() which permits
them to use arch-specific log calculation instructions - such as BSR on
x86/x86_64 or SCAN on FRV - if available.

David Howells 65f27f3844 WorkStruct: Pass the work_struct pointer instead of context data
Pass the work_struct pointer to the work function rather than context data.
The work function can use container_of() to work out the data.

For the cases where the container of the work_struct may go away the moment the
pending bit is cleared, it is made possible to defer the release of the
structure by deferring the clearing of the pending bit.

To make this work, an extra flag is introduced into the management side of the
work_struct. This governs auto-release of the structure upon execution.

Ordinarily, the work queue executor would release the work_struct for further
scheduling or deallocation by clearing the pending bit prior to jumping to the
work function. This means that, unless the driver makes some guarantee itself
that the work_struct won't go away, the work function may not access anything
else in the work_struct or its container lest they be deallocated.. This is a
problem if the auxiliary data is taken away (as done by the last patch).

However, if the pending bit is *not* cleared before jumping to the work
function, then the work function *may* access the work_struct and its container
with no problems. But then the work function must itself release the
work_struct by calling work_release().

In most cases, automatic release is fine, so this is the default. Special
initiators exist for the non-auto-release case (ending in _NAR).

David Howells c636ebdb18 [PATCH] VFS: Destroy the dentries contributed by a superblock on unmounting
The attached patch destroys all the dentries attached to a superblock in one go
by:

 (1) Destroying the tree rooted at s_root.

 (2) Destroying every entry in the anon list, one at a time.

 (3) Each entry in the anon list has its subtree consumed from the leaves
     inwards.

This reduces the amount of work generic_shutdown_super() does, and avoids
iterating through the dentry_unused list.

Note that locking is almost entirely absent in the shrink_dcache_for_umount*()
functions added by this patch. This is because:

 (1) at the point the filesystem calls generic_shutdown_super(), it is not
     permitted to further touch the superblock's set of dentries, and nor may
     it remove aliases from inodes;

 (2) the dcache memory shrinker now skips dentries that are being unmounted;
     and

 (3) the superblock no longer has any external references through which the VFS
     can reach it.

Given these points, the only locking we need to do is when we remove dentries
from the unused list and the name hashes, which we do a directory's worth at a
time.

We also don't need to guard against reference counts going to zero unexpectedly
and removing bits of the tree we're working on as nothing else can call dput().

A cut down version of dentry_iput() has been folded into
shrink_dcache_for_umount_subtree() function. Apart from not needing to unlock
things, it also doesn't need to check for inotify watches.

In this version of the patch, the complaint about a dentry still being in use
has been expanded from a single BUG_ON() and now gives much more information.

David Howells 7d12e780e0 IRQ: Maintain regs pointer globally rather than passing to IRQ handlers
Maintain a per-CPU global "struct pt_regs *" variable which can be used instead
of passing regs around manually through all ~1800 interrupt handlers in the
Linux kernel.

The regs pointer is used in few places, but it potentially costs both stack
space and code to pass it around. On the FRV arch, removing the regs parameter
from all the genirq function results in a 20% speed up of the IRQ exit path
(ie: from leaving timer_interrupt() to leaving do_IRQ()).

Where appropriate, an arch may override the generic storage facility and do
something different with the variable. On FRV, for instance, the address is
maintained in GR28 at all times inside the kernel as part of general exception
handling.

Having looked over the code, it appears that the parameter may be handed down
through up to twenty or so layers of functions. Consider a USB character
device attached to a USB hub, attached to a USB controller that posts its
interrupts through a cascaded auxiliary interrupt controller. A character
device driver may want to pass regs to the sysrq handler through the input
layer which adds another few layers of parameter passing.

I've build this code with allyesconfig for x86_64 and i386. I've runtested the
main part of the code on FRV and i386, though I can't test most of the drivers.
I've also done partial conversion for powerpc and MIPS - these at least compile
with minimal configurations.

This will affect all archs. Mostly the changes should be relatively easy.
Take do_IRQ(), store the regs pointer at the beginning, saving the old one:

	struct pt_regs *old_regs = set_irq_regs(regs);

And put the old one back at the end:

	set_irq_regs(old_regs);

Don't pass regs through to generic_handle_irq() or __do_IRQ().

In timer_interrupt(), this sort of change will be necessary:

	-	update_process_times(user_mode(regs));
	-	profile_tick(CPU_PROFILING, regs);
	+	update_process_times(user_mode(get_irq_regs()));
	+	profile_tick(CPU_PROFILING);

I'd like to move update_process_times()'s use of get_irq_regs() into itself,
except that i386, alone of the archs, uses something other than user_mode().

Some notes on the interrupt handling in the drivers:

 (*) input_dev() is now gone entirely. The regs pointer is no longer stored in
     the input_dev struct.

 (*) finish_unlinks() in drivers/usb/host/ohci-q.c needs checking. It does
     something different depending on whether it's been supplied with a regs
     pointer or not.

 (*) Various IRQ handler function pointers have been moved to type
     irq_handler_t.

David Howells afefdbb28a [PATCH] VFS: Make filldir_t and struct kstat deal in 64-bit inode numbers
These patches make the kernel pass 64-bit inode numbers internally when
communicating to userspace, even on a 32-bit system. They are required
because some filesystems have intrinsic 64-bit inode numbers: NFS3+ and XFS
for example. The 64-bit inode numbers are then propagated to userspace
automatically where the arch supports it.

Problems have been seen with userspace (eg: ld.so) using the 64-bit inode
number returned by stat64() or getdents64() to differentiate files, and
failing because the 64-bit inode number space was compressed to 32-bits, and
so overlaps occur.

This patch:

Make filldir_t take a 64-bit inode number and struct kstat carry a 64-bit
inode number so that 64-bit inode numbers can be passed back to userspace.

The stat functions then returns the full 64-bit inode number where
available and where possible. If it is not possible to represent the inode
number supplied by the filesystem in the field provided by userspace, then
error EOVERFLOW will be issued.

Similarly, the getdents/readdir functions now pass the full 64-bit inode
number to userspace where possible, returning EOVERFLOW instead when a
directory entry is encountered that can't be properly represented.

Note that this means that some inodes will not be stat'able on a 32-bit
system with old libraries where they were before - but it does mean that
there will be no ambiguity over what a 32-bit inode number refers to.

Note similarly that directory scans may be cut short with an error on a
32-bit system with old libraries where the scan would work before for the
same reasons.

It is judged unlikely that this situation will occur because modern glibc
uses 64-bit capable versions of stat and getdents class functions
exclusively, and that older systems are unlikely to encounter
unrepresentable inode numbers anyway.

David Howells 3f2e05e90e [PATCH] BLOCK: Revert patch to hack around undeclared sigset_t in linux/compat.h
Revert Andrew Morton's patch to temporarily hack around the lack of a
declaration of sigset_t in linux/compat.h to make the block-disablement
patches build on IA64. This got accidentally pushed to Linus and should
be fixed in a different manner.

Also make linux/compat.h #include asm/signal.h to gain a definition of
sigset_t so that it can externally declare sigset_from_compat().

This has been compile-tested for i386, x86_64, ia64, mips, mips64, frv, ppc and
ppc64 and run-tested on frv.

David Howells 65934a9a02 [PATCH] BLOCK: Make USB storage depend on SCSI rather than selecting it [try #6]
This makes CONFIG_USB_STORAGE depend on CONFIG_SCSI rather than selecting it,
as selecting it makes CONFIG_USB_STORAGE override the dependencies of SCSI,
causing it to turn on even if they aren't all met.

David Howells 9361401eb7 [PATCH] BLOCK: Make it possible to disable the block layer [try #6]
Make it possible to disable the block layer. Not all embedded devices require
it, some can make do with just JFFS2, NFS, ramfs, etc - none of which require
the block layer to be present.

This patch does the following:

 (*) Introduces CONFIG_BLOCK to disable the block layer, buffering and blockdev
     support.

 (*) Adds dependencies on CONFIG_BLOCK to any configuration item that controls
     an item that uses the block layer. This includes:

     (*) Block I/O tracing.

     (*) Disk partition code.

     (*) All filesystems that are block based, eg: Ext3, ReiserFS, ISOFS.

     (*) The SCSI layer. As far as I can tell, even SCSI chardevs use the
     	 block layer to do scheduling. Some drivers that use SCSI facilities -
     	 such as USB storage - end up disabled indirectly from this.

     (*) Various block-based device drivers, such as IDE and the old CDROM
     	 drivers.

     (*) MTD blockdev handling and FTL.

     (*) JFFS - which uses set_bdev_super(), something it could avoid doing by
     	 taking a leaf out of JFFS2's book.

 (*) Makes most of the contents of linux/blkdev.h, linux/buffer_head.h and
     linux/elevator.h contingent on CONFIG_BLOCK being set. sector_div() is,
     however, still used in places, and so is still available.

 (*) Also made contingent are the contents of linux/mpage.h, linux/genhd.h and
     parts of linux/fs.h.

 (*) Makes a number of files in fs/ contingent on CONFIG_BLOCK.

 (*) Makes mm/bounce.c (bounce buffering) contingent on CONFIG_BLOCK.

 (*) set_page_dirty() doesn't call __set_page_dirty_buffers() if CONFIG_BLOCK
     is not enabled.

 (*) fs/no-block.c is created to hold out-of-line stubs and things that are
     required when CONFIG_BLOCK is not set:

     (*) Default blockdev file operations (to give error ENODEV on opening).

 (*) Makes some /proc changes:

     (*) /proc/devices does not list any blockdevs.

     (*) /proc/diskstats and /proc/partitions are contingent on CONFIG_BLOCK.

 (*) Makes some compat ioctl handling contingent on CONFIG_BLOCK.

 (*) If CONFIG_BLOCK is not defined, makes sys_quotactl() return -ENODEV if
     given command other than Q_SYNC or if a special device is specified.

 (*) In init/do_mounts.c, no reference is made to the blockdev routines if
     CONFIG_BLOCK is not defined. This does not prohibit NFS roots or JFFS2.

 (*) The bdflush, ioprio_set and ioprio_get syscalls can now be absent (return
     error ENOSYS by way of cond_syscall if so).

 (*) The seclvl_bd_claim() and seclvl_bd_release() security calls do nothing if
     CONFIG_BLOCK is not set, since they can't then happen.
