With error logging now completely handled in
the op_mode, the transport layer does not
need to know information about the loaded
firmware.
Remove this state information from the
iwl_shared data structure.

This variable holds the ucode currently
running on the device; which is determined by
op_mode, so move this parameter there.
Also, the name of the variable is a bit
misleading, so rename it to cur_ucode.

Error log reporting does not belong to the
transport layer, but to the op_mode loading
the ucode, as it is the entity which knows
about the ucode loaded, and what the error
information means.
Move device logging pointers from the
transport layer to op_mode.
With this change, transport layer only
reports an error to the op_mode, which will
figure out what to do with the error. This
causes the driver to now dump out error logs
when the command queue is stuck as well.
Also, move the debugfs entry for event logs
out of the transport layer and into op_mode.

This wait queue really belongs to the transport
layer, as it is used for sending synchronous
commands to the HW.
However, only op_mode knows about errors and
exceptional conditions, so make this queue
accessible by the op_mode.

The command queue number is required by the transport
layer, but it can be determined only by the op mode.

If the firmware implements beacon filtering,
beacons are filtered when not associated. This causes
association failures on channels marked passive.
Enabling this flag indicates to the firmware to allow
the beacons to pass through when not associated only.

Runtime DC calibration was previously conditional. Remove this
behaviour, as new devices support runtime DC calibration,
while older devices ignore the runtime DC calibration request.
This patch addresses low TX throughput issues seen with the 6205.

iwlwifi tries to avoid using antenna B in BT combo devices when BT
is active. A bug in the rate-scaling algorithm was causing the combo
device to never attempt MIMO rates. Fix the algorithm to
opportunistically try MIMO rates when BT traffic is low.

WiFi throughput drops drastically when BT is turned on, BT and WiFi
are simultaneously transmitting/receiving traffic. This is particularly true
when BT has higher priority over WiFi, and hence the device defers TX frames.
The AP assumes that the channel is bad and reduces the data rate, implying
longer airtime, which exacerbates the problem further, resulting ultimately
in what is popularly called the "death-spiral" phenomenon. The use of PS-poll
in such scenarios guarantees a low but consistent throughput.
Since the death-spiral phenomenon is observed only when the RSSI is low, use
PS-poll only when RSSI is low and disable when high, with a known hysterisis.
This feature specifies the high and low thresholds and implements the
callbacks registered with mac80211, which will be called when threshold events
occur.

mac80211 maintains a running average of the RSSI when a STA
is associated to an AP. Report threshold events to any driver
that has registered callbacks for getting RSSI measurements.
Implement callbacks in mac80211 so that driver can set thresholds.
Add callbacks in mac80211 which is invoked when an RSSI threshold
event occurs.
mac80211: add tracing to rssi_reports api and remove extraneous fn argument
mac80211: scale up rssi thresholds from driver by 16 before storing

The iwlagn powersave algorithm uses the supports_idle parameter
to tell the device to save power when it is not associated with
an AP and is idle.
Enable this feature for the 1000 series of devices.
