Change proc_dir_entry->size to be loff_t to represent files like
/proc/vmcore for 32bit systems with more than 4G memory.
Needed for seeing correct size for /proc/vmcore for 32-bit systems with >
4G RAM.

lapic_shutdown() re-enables interrupts which is un-desirable for panic
case, so use local_irq_save() and local_irq_restore() to keep the irqs
disabled for kexec on panic case, and close a possible race window while
kdump shutdown as shown in this stack trace

I have heard some complaints about people not finding CONFIG_CRASH_DUMP
option and also some objections about its dependency on CONFIG_EMBEDDED.
The following patch ends that dependency. I thought of hiding it under
CONFIG_KEXEC, but CONFIG_PHYSICAL_START could also be used for some reasons
other than kexec/kdump and hence left it visible. I will also update the
documentation accordingly.
o Following patch removes the config dependency of CONFIG_PHYSICAL_START
on CONFIG_EMBEDDED. The reason being CONFIG_CRASH_DUMP option for
kdump needs CONFIG_PHYSICAL_START which makes CONFIG_CRASH_DUMP depend
on CONFIG_EMBEDDED. It is not always obvious for kdump users to choose
CONFIG_EMBEDDED.
o It also shifts the palce where this option appears, to make it closer
to kexec and kdump options.

This moves the code to free devt_attr from class_device_del() to
class_dev_release() which is called after the last reference to the
corresponding kobject() is gone.
This allows us to keep the devt_attr alive while the corresponding
sysfs file is open.

o sysfs_dirent's s_mode field should also be updated in sysfs_setattr(), else
there could be inconsistency in the two fields. s_mode is used while
->readdir so as not to bring in the inode to cache.

o Following patch sets the attributes for newly allocated inodes for sysfs
objects. If the object has non-default attributes, inode attributes are
set as saved in sysfs_dirent->s_iattr, pointer to struct iattr.

o This adds ->i_op->setattr VFS method for sysfs inodes. The changed
attribues are saved in the persistent sysfs_dirent structure as a pointer
to struct iattr. The struct iattr is allocated only for those sysfs_dirent's
for which default attributes are getting changed. Thanks to Jon Smirl for
this suggestion.

o The following patch makes sure to attach sysfs_dirent to the dentry before
allocation a new inode through sysfs_create(). This change is done as
preparatory work for implementing ->i_op->setattr() functionality for
