Corey Minyard f64da958df ipmi: add new IPMI nmi watchdog handling
Convert over to the new NMI handling for getting IPMI watchdog timeouts via an
NMI.  This add config options to know if there is the ability to receive NMIs
and if it has an NMI post processing call.  Then it modifies the IPMI watchdog
to take advantage of this so that it can know if an NMI comes in.

It also adds testing that the IPMI NMI watchdog works.

Corey Minyard ee6cd5f8f5 ipmi: allow shared interrupts
The IPMI driver used enable_irq and disable_irq when it got into situations
where it couldn't allocate memory; it did this to avoid having the interrupt
just lock the machine when it couldn't get memory to perform the transaction
to disable the interrupt.

This patch modifies the driver to not use disable_irq and enable_irq.  It
instead sends the messages to the BMC to perform this operation.  It also
makes sure interrupts are cleanly disabled when the interface is shut down and
cleans up some shutdown things that are no longer necessary.

Corey Minyard dba9b4f6a0 ipmi: add powerpc openfirmware sensing
Add support for of_platform_driver to the ipmi_si module.  When loading the
module, the driver will be registered to of_platform.  The driver will be
probed for all devices with the type ipmi.  It's supporting devices with
compatible settings ipmi-kcs, ipmi-smic and ipmi-bt.  Only ipmi-kcs could be
tested.

Corey Minyard 78ba2faf71 [PATCH] IPMI: Fix some RCU problems
Fix some RCU problem pointed out by Paul McKenney of IBM.  These are:

The wholesale move of the command receivers list into a new list was not
safe because the list will point to the new tail during a traversal, so the
traversal will never end on a reader if this happens during a read.

Memory barriers were needed to handle proper ordering of the setting of the
IPMI interface as valid.  Readers might not see proper ordering of data
otherwise.

In ipmi_smi_watcher_register(), the use of the _rcu suffix on the list is
unnecessary.

This require the list_splice_init_rcu() patch previously posted.

Corey Minyard 3678d62f02 [PATCH] add an RCU version of list splicing
This patch is in support of the IPMI driver.  I have tested this with the
IPMI driver changes coming in the next patch.

Add a list_splice_init_rcu() function to splice an RCU-protected list into
another list.  This takes the sync function as an argument, so one would do
something like:

	INIT_LIST_HEAD(&list);
	list_splice_init_rcu(&source, &dest, synchronize_rcu);

The idea being to keep the RCU API proliferation down to a dull roar.

Fix various problems pointed out by Andrew Morton and others:
  * platform_device_unregister checks for NULL, no need to check here.
  * Formatting fixes.
  * Remove big macro and convert to a function.
  * Use strcmp instead of defining a broken case-insensitive comparison,
    and make the output parameter info match the case of the input one
    (change "I/O" to "i/o").
  * Return the length instead of 0 from the hotmod parameter handler.
  * Remove some unused cruft.
  * The trydefaults parameter only has to do with scanning the "standard"
    addresses, don't check for that on ACPI.

Corey Minyard 4d7cbac7c8 [PATCH] IPMI: Fix BT long busy
The IPMI BT subdriver has been patched to survive "long busy" timeouts seen
during firmware upgrades and resets.  The patch never returns the HOSED state,
synthesizes response messages with meaningful completion codes, and recovers
gracefully when the hardware finishes the long busy.  The subdriver now issues
a "Get BT Capabilities" command and properly uses those results.  More
informative completion codes are returned on error from transaction starts;
this logic was propogated to the KCS and SMIC subdrivers.  Finally, indent and
other style quirks were normalized.

Corey Minyard 55162fb165 [PATCH] IPMI: fix request events
When the IPMI message handler requested that the interface look for events,
the ipmi_si driver would request flags, see if the event buffer full flag was
set, then request events.  It's better to just send the request for events, as
it cuts one message out of the transaction if there happens to be events, and
it will fetch events even if the event buffer was not full.

Corey Minyard b9675136e2 [PATCH] IPMI: Add maintenance mode
Some commands and operations on a BMC can cause the BMC to "go away" for a
while.  This can cause the automatic flag processing and other things of that
nature to timeout and generate annoying logs, or possibly cause other bad
things to happen when in firmware update mode.

Add detection of those commands (cold reset, warm reset, and any firmware
command) and turns off automatic processing for 30 seconds.  It also add a
manual override either way.

This modifies the IPMI driver so that a lower-level interface can be
dynamically removed while in use so it can support hot-removal of hardware.

It also adds the ability to specify and dynamically change the IPMI interface

Corey Minyard f0b55da0d2 [PATCH] IPMI: Fix device model name
Add the product id to the driver model platform device name, in addition to
the device id.  The IPMI spec does not require that individual BMCs in a
system have unique devices IDs, but it does require that the product id/device
id combination be unique.

This also removes a redundant check and cleans up error handling
when the sysfs registration fails.

A wrong function was being used to free a list; this fixes the problem.
Otherwise, an oops at unload time was possible.  But not likely, since you
can't have any users when you unload the modules and it is very hard to get
messages into this queue without users.

Corey Minyard 469b1d8741 [PATCH] x86-64: Fix for arch/x86_64/pci/Makefile CFLAGS
The arch/x86_64/pci directory was giving problems in a wierd cross-compile
environment.  The exact cause is unknown, but the Makefile used CFLAGS
instead of EXTRA_CFLAGS.  From what I can tell from
Documentation/kbuild/makefiles.txt, CFLAGS should not be used for this, it
should be EXTRA_CFLAGS.  And it solves the cross-compile problem.

Corey Minyard a51f4a81e7 [PATCH] IPMI: allow user to override the kernel IPMI daemon enable
After the previous patch to disable the kernel IPMI daemon if interrupts
were available, the issue of broken hardware was raised, and a reasonable
request to add an override was mode.  So here it is.

Allow the user to force the kernel ipmi daemon on or off.  This way,
hardware with broken interrupts or users that are not concerned with
performance can turn it on or off to their liking.

Corey Minyard df3fe8defe [PATCH] ipmi: don't start kipmid if the IPMI driver can use interrupts
If the driver has interrupts available to it, there is really no reason to
have a kernel daemon push the IPMI state machine.

Note that I have experienced machines where the interrupts do not work
correctly.  This was a long time ago and hopefully things are better now.
If some machines still have broken interrupts, a blacklist will need to be
added.

Corey Minyard c69c31270c [PATCH] IPMI: per-channel command registration
This patch adds the ability to register for a command per-channel in the
IPMI driver.

If your BMC supports multiple channels, incoming messages can be useful to
have the ability to register to receive commands on a specific channel
instead the current behaviour of all channels.

Corey Minyard 4064d5ef26 [PATCH] IPMI: fix handling of OEM flags
If one of the OEM flags becomes set in the flags from the hardware, the
driver could hang if no OEM handler was set.  Fix the code to handle this.
This was tested by setting the flags by hand after they were fetched.

Allow the pm_power_off function variable in PPC to work as an override.
This makes the function consistent with the other architectures and it
allows generic poweroff operations (like those provided in IPMI
systems) to work properly on PPC.

Corey Minyard a9eec55623 [PATCH] IPMI: fix occasional oops on module unload
Olaf Kirch of SuSE tracked down a problem where module unloads of the IPMI
driver would occasionally result in Oopses.  He tracked that down to a
variable that wasn't always initialized properly in some situations.  This
patch initializes that variable.  Olaf sent a patch that kzalloc-ed the
data, but this structure is large enough that I would perfer to not do
that.  Thanks Olaf!

Corey Minyard 96febe9fb7 [PATCH] IPMI: watchdog handle panic properly
Modify the watchdog timeout in IPMI to only do things at panic/reboot time if
the watchdog timer was already running.  Some BIOSes do not disable the
watchdog timer at startup, and this led to a reboot a while later if the new
OS running didn't start monitoring the watchdog, even if the watchdog was not
running before.

Corey Minyard e05b59fe79 [WATCHDOG] Pre-Timeout flags
Some watchdog timers support the concept of a "pretimeout" which
occurs some time before the real timeout.  The pretimeout can
be delivered via an interrupt or NMI and can be used to panic
the system when it occurs (so you get useful information instead
of a blind reboot).

This patch is pretty important to get in for IPMI, new systems have been
changing the way ACPI and IPMI interact, and this works around the problems
for now.  This is a temporary fix until we get proper ACPI handling in
IPMI.

Fixed releasing already-allocated regions when a later request fails, and
forward-ported it to HEAD.

Some BIOSes reserve disjoint I/O regions in their ACPI tables for the IPMI
controller.  This causes problems when trying to register the entire I/O
region.  Therefore we must register each I/O port separately.

Corey Minyard 8b1ffe9550 [PATCH] x86_64: add nmi_exit to die_nmi
Playing with NMI watchdog on x86_64, I discovered that it didn't
do what I expected.  It always panic-ed, even when it didn't
happen from interrupt context.  This patch solves that
problem for me.  Also, in this case, do_exit() will be called
with interrupts disabled, I believe.  Would it be wise to also
call local_irq_enable() after nmi_exit()?
[Yes I added it -AK]

Currently, on x86_64, any NMI watchdog timeout will cause a panic
because the irq count will always be set to be in an interrupt
when do_exit() is called from die_nmi().  If we add nmi_exit() to
the die_nmi() call (since the nmi will never exit "normally")
it seems to solve this problem.  The following small program
can be used to trigger the NMI watchdog to reproduce this:

Corey Minyard cdc60a4c8e [PATCH] x86_64: fix die_lock nesting
I noticed this when poking around in this area.

The oops_begin() function in x86_64 would only conditionally claim
the die_lock if the call is nested, but oops_end() would always
release the spinlock. This patch adds a nest count for the die lock
so that the release of the lock is only done on the final oops_end().

Corey Minyard 4791c03d2c [PATCH] ipmi: fix event queue limit
The event handler mechanism in the IPMI driver had a limit on the number of
received events, but the counts were not being updated.  Update the counts
to impose a limit.  This is not a critical fix, as this function (the
sending of the events) has to be turned on by the user, anyway.  This
avoids problems if they forget to turn it back off.

Corey Minyard 453823ba08 [PATCH] IPMI: fix startup race condition
Matt Domsch noticed a startup race with the IPMI kernel thread, it was
possible (though extraordinarly unlikely) that a message could come in
before the upper layer was ready to handle it.  This patch splits the
startup processing of an IPMI interface into two parts, one to get ready
and one to actually start the processes to receive messages from the
interface.

Add full driver model support for the IPMI driver.  It links in the proper
bus and device support.

It adds an "ipmi" driver interface that has each BMC discovered by the
driver (as a device).  These BMCs appear in the devices/platform directory.
 If there are multiple interfaces to the same BMC, the driver should
discover this and will only have one BMC entry.  The BMC entry will have
pointers to each interface device that connects to it.

The device information (statistics and config information) has not yet been
ported over to the driver model from proc, that will come later.

This work was based on work by Yani Ioannou.  I basically rewrote it using
that code as a guide, but he still deserves credit :).

If a panic came from the IPMI watchdog pretimeout and that was reported via
an NMI, it would also be reported via the standard IPMI flags, which would
get picked up when reporting panic events and cause another panic.  This

Corey Minyard a9a2c44ff0 [PATCH] ipmi: add timer thread
We must poll for responses to commands when interrupts aren't in use.  The
default poll interval is based on using a kernel timer, which varies with HZ.
For character-based interfaces like KCS and SMIC though, that can be way too
slow (>15 minutes to flash a new firmware with KCS, >20 seconds to retrieve
the sensor list).

This creates a low-priority kernel thread to poll more often.  If the state
machine is idle, so is the kernel thread.  But if there's an active command,
it polls quite rapidly.  This decrease a firmware flash time from 15 minutes
to 1.5 minutes, and the sensor list time to 4.5 seconds, on a Dell PowerEdge
x8x system.

The timer-based polling remains, to ensure some amount of responsiveness even
under high user process CPU load.

Checking for a stopped timer at rmmod now uses atomics and del_timer_sync() to
ensure safe stoppage.

Corey Minyard c3e7e7916e [PATCH] ipmi: kcs error0 delay
BMCs can get into ERROR0 state while flashing new firmware, particularly while
the BMC is erasing the next flash block, which may take a just under 2 seconds
on a Dell PowerEdge 2800 (1.75 seconds typical), during which time the
single-threaded firmware may not be able to process new commands.  In
particular, clearing OBF may not take effect immediately.

We want it to delay in ERROR0 after clearing OBF a bit waiting for OBF to
actually be clear before proceeding.

This introduces a new return value from the LLDD's event loop,
SI_SM_CALL_WITH_TICK_DELAY.  This means the calling thread/timer should
schedule_timeout() at least 1 tick, rather than busy-wait.  This is a longer
delay than SI_SM_CALL_WITH_DELAY, which is typically a 250us busy-wait.

Corey Minyard 21dcd300b1 [PATCH] ipmi: bt restart reset fixes
The current BT retry/reset mechanism fails to succeed on a PowerEdge 1650,
when the controller is wedged with B2H_ATN asserted at XACTION_START.  If this
occurs, no further commands will ever succeed unless the state of the
controller is first cleared out.

Furthermore, the soft reset would only occur if the first command after insmod
was the one that timed out, not if a later command timed out.

This patch changes the retry/reset mechanism to be as follows:

Before retrying a command, clear the state of the BT controller such that the
flags represent ready for a new transaction.  This increases the chance of
success of the restarted transaction.

After 2 retries, issue a soft reset and retry one more time before giving up
and reporting back a failure.

Corey Minyard ea94027b92 [PATCH] ipmi: si start transaction hook
Some commands, on some system BMCs, don't respond at at all.  This is seen on
Dell PowerEdge x6xx and x7xx systems with IPMI 1.0 BT controllers when a "Get
SDR" command is issued, with a length field of 0x3A, which happens to be the
length of about SDR entries.  If another length is passed, this command
succeeds.

This patch adds general infrastructure for receiving commands before they're
passed down to the low-level drivers, such that they can be completed
immediately, or modified, prior to being sent to ->start_transaction().

Corey Minyard c4edff1c19 [PATCH] ipmi: various si cleanup
A number of small changes for the various system interface drivers,
consolidated from a number of patches from Matt Domsch.

Clear B2H_ATN and drain the BMC message buffer on command timeout.  This
prevents further commands from failing after a timeout.

Add bt_debug and smic_debug module parameters, expose them in sysfs.  This
lets you enable and disable debugging messages at runtime.

Unsigned jiffies math in ipmi_si_intf.c causes a too-large value to be passed
to ->event() after jiffies wrap-around.  The BT driver had caught this, but
didn't know how to fix it.  Now all calls to ->event() use a sane value for
time.

Increase timeout for commands handed to the BT driver from 2 seconds to 5
seconds.  This is necessary particularly when the previous command was a
"Clear SEL", as that command completes, yet the BMC isn't really ready to
handle another command yet.

Silence BT debugging messages which were being printed on the console.

Increase SMIC timeout form 1/10s to 2s.  This is needed on Dell PowerEdge 2650
and PowerEdge 750 with ERA/O cards to allow commands to complete without
timing out.

Adds kcs_debug module param, to match behavior of BT and SMIC.  This also
prevents messages from being sent to the console unless explicitly requested.

Corey Minyard 393d2cc354 [PATCH] ipmi: use refcount in message handler
This patch is rather large, but it really can't be done in smaller chunks
easily and I believe it is an important change.  This has been out and tested
for a while in the latest IPMI driver release.  There are no functional
changes, just changes as necessary to convert the locking over (and a few
minor style updates).

The IPMI driver uses read/write locks to ensure that things exist while they
are in use.  This is bad from a number of points of view.  This patch removes
the rwlocks and uses refcounts and RCU lists to manage what the locks did.

Corey Minyard 168524d673 [PATCH] ipmi: add hacks for IPMI chassis poweroff for certain Dell servers
This patch allows Dell servers with IPMI controllers that predate IPMI 1.5
to use the standard poweroff or powercycle commands.  These systems
firmware don't set the chassis capability bit in the Get Device ID, but
they do implement the standard poweroff and powercycle commands.

Tested on RHEL3 kernel 2.4.21-20.ELsmp on a PowerEdge 2600.  The standard
ipmi_poweroff driver cannot drive these systems.  With this patch, they
power off or powercycle as expected.

Corey Minyard 3ae0e0f9b1 [PATCH] ipmi: OEM flag handling and hacks for some Dell machines
The ipmi driver does not have a way to handle firmware-generated events
which have the OEM[012] Data Available flags set.  In such a case, the
SMS_ATN bit may never get cleared by firmware, leaving the driver looping
infinitely but never able to make any progress.

This patch first simplifies storage and use of the data returned from an
IPMI Get Device ID command.

It then creates a new per-OEM handler hook, which should know how to handle
events with the OEM[012] Data Available flags set.  It then uses this to
implement a workaround for IPMI 1.5-capable Dell PowerEdge servers which
are susceptable to setting the OEM[012] Data Available flags when the
driver can't handle it.

Corey Minyard c14979b993 [PATCH] ipmi: add per-channel IPMB addresses
IPMI allows multiple IPMB channels on a single interface, and each channel
might have a different IPMB address.  However, the driver has only one IPMB
address that it uses for everything.  This patch adds new IOCTLS and a new
internal interface for setting per-channel IPMB addresses and LUNs.  New
systems are coming out with support for multiple IPMB channels, and they are
broken without this patch.

Corey Minyard 3b6259432d [PATCH] ipmi: add power cycle capability
This patch to adds "power cycle" functionality to the IPMI power off module
ipmi_poweroff.  It also contains changes to support procfs control of the
feature.

The power cycle action is considered an optional chassis control in the IPMI
specification.  However, it is definitely useful when the hardware supports
it.  A power cycle is usually required in order to reset a firmware in a bad
state.  This action is critical to allow remote management of servers.

The implementation adds power cycle as optional to the ipmi_poweroff module.
It can be modified dynamically through the proc entry mentioned above.  During
a power down and enabled, the power cycle command is sent to the BMC firmware.
 If it fails either due to non-support or some error, it will retry to send
the command as power off.

Enable interrupts for a BT interface.  There is a specific register that
needs to be set up to enable interrupts that also must be modified to clear
the irq.

Also, don't reset the BMC on a BT interface.  That's probably not a good
idea as the BMC may be performing other important functions and a reset
should only be a last resort.  Also, that register is also used to
enable/disable interrupts to the BT; modifying it may screw up the
interrupts.
