We have a kernel build regression since 3.1-rc1, which is about 10%
regression. The kernel source is in an ext3 filesystem.

Apparently this is caused by lack metadata preemption, where ext3/ext4
do use READ_META. I didn't see a way to fix the issue, so suggest

Fix this by just using raw_smp_processor_id(), it's just a hint
after all. There's no pinning of the CPU or accessing per-cpu
structures involved.

Since we are modifying this RCU pointer, we need to hold
the lock protecting it around it.

This fixes a potential reuse and double free of a cfq
io_context structure. The bug has been in CFQ for a long
time, it hit very few people but those it did hit seemed
to see it a lot.

Jens Axboe a1706ac4c0 Revert "block: Remove extra discard_alignment from hd_struct."
It was not a good idea to start dereferencing disk->queue from
the fs sysfs strategy for displaying discard alignment. We ran
into first a NULL pointer deref, and after fixing that we sometimes
see unvalid disk->queue pointer values.

Since discard is the only one of the bunch actually looking into
the queue, just revert the change.

This reverts commit 23ceb5b771.

Not quite known how ->queue can be NULL while the sysfs entry

Since for-2.6.40/core was forked off the 2.6.39 devel tree, we've
had churn in the core area that makes it difficult to handle
patches for eg cfq or blk-throttle. Instead of requiring that they
be based in older versions with bugs that have been fixed later
in the rc cycle, merge in 2.6.39 final.

Also fixes up conflicts in the below files.

Jens Axboe 9937a5e2f3 scsi: remove performance regression due to async queue run
Commit c21e6beb removed our queue request_fn re-enter
protection, and defaulted to always running the queues from
kblockd to be safe. This was a known potential slow down,
but should be safe.

Unfortunately this is causing big performance regressions for
some, so we need to improve this logic. Looking into the details
of the re-enter, the real issue is on requeue of requests.

Requeue of requests upon seeing a BUSY condition from the device
ends up re-running the queue, causing traces like this:

scsi_request_fn()
        scsi_dispatch_cmd()
                scsi_queue_insert()
                        __scsi_queue_insert()
                                scsi_run_queue()
					scsi_request_fn()
						...

potentially causing the issue we want to avoid. So special
case the requeue re-run of the queue, but improve it to offload
the entire run of local queue and starved queue from a single
workqueue callback. This is a lot better than potentially
kicking off a workqueue run for each device seen.

This also fixes the issue of the local device going into recursion,
since the above mentioned commit never moved that queue run out
of line.

Jens Axboe 3aa72873ff elevator: check for ELEVATOR_INSERT_SORT_MERGE in !elvpriv case too
The sort insert is the one that goes to the IO scheduler. With
the SORT_MERGE addition, we could bypass IO scheduler setup
but still ask the IO scheduler to insert the request. This would
cause an oops on switching IO schedulers through the sysfs
interface, unless the disk just happened to be idle while it
occured.

Jens Axboe c21e6beba8 block: get rid of QUEUE_FLAG_REENTER
We are currently using this flag to check whether it's safe
to call into ->request_fn(). If it is set, we punt to kblockd.
But we get a lot of false positives and excessive punts to
kblockd, which hurts performance.

The only real abuser of this infrastructure is SCSI. So export
the async queue run and convert SCSI over to use that. There's
room for improvement in that SCSI need not always use the async
call, but this fixes our performance issue and they can fix that
up in due time.

Jens Axboe 5f45c69589 cfq-iosched: read_lock() does not always imply rcu_read_lock()
For some configurations of CONFIG_PREEMPT that is not true. So
get rid of __call_for_each_cic() and always uses the explicitly
rcu_read_lock() protected call_for_each_cic() instead.

This fixes a potential bug related to IO scheduler removal or
online switching.

Jens Axboe 99e22598e9 block: drop queue lock before calling __blk_run_queue() for kblockd punt
If we know we are going to punt to kblockd, we can drop the queue
lock before calling into __blk_run_queue() since it only does a
safe bit test and a workqueue call. Since kblockd needs to grab
this very lock as one of the first things it does, it's a good
optimization to drop the lock before waking kblockd.

Jens Axboe 49cac01e1f block: make unplug timer trace event correspond to the schedule() unplug
It's a pretty close match to what we had before - the timer triggering
would mean that nobody unplugged the plug in due time, in the new
scheme this matches very closely what the schedule() unplug now is.
It's essentially the difference between an explicit unplug (IO unplug)
or an implicit unplug (timer unplug, we scheduled with pending IO
queued).

Jens Axboe a237c1c5bc block: let io_schedule() flush the plug inline
Linus correctly observes that the most important dispatch cases
are now done from kblockd, this isn't ideal for latency reasons.
The original reason for switching dispatches out-of-line was to
avoid too deep a stack, so by _only_ letting the "accidental"
flush directly in schedule() be guarded by offload to kblockd,
we should be able to get the best of both worlds.

So add a blk_schedule_flush_plug() that offloads to kblockd,
and only use that from the schedule() path.

For the explicit unplugging, we'd prefer to kick things off
immediately and not pay the penalty of the latency to switch
to kblockd. So let blk_finish_plug() do the run inline, while
the implicit-on-schedule-out unplug will punt to kblockd.

There are worries that we are now consuming a lot more stack in
some cases, since we potentially call into IO dispatch from
schedule() or io_schedule(). We can reduce this problem by moving
the running of the queue to kblockd, like the old plugging scheme
did as well.

This may or may not be a good idea from a performance perspective,
depending on how many tasks have queue plugs running at the same
time. For even the slightly contended case, doing just a single
queue run from kblockd instead of multiple runs directly from the
unpluggers will be faster.

Jens Axboe 782b86e265 ide: always ensure that blk_delay_queue() is called if we have pending IO
Just because we are not requeuing a request does not mean that
some aren't pending. So always issue a blk_delay_queue() if
either we are requeueing OR there's pending IO.

This fixes a boot problem for some IDE boxes.

With the ->sync_page() hook gone, we have a few users that
add their own static address_space_operations without any
functions defined.

fs/inode.c already has an empty_aops that it uses for init
purposes. Lets export that and use it in the places where
an otherwise empty aops was defined.

Jens Axboe 9ced0b95b3 ide: ensure that we re-run the queue handler
The conversion to blk_delay_queue() missed parts of IDE.
Add a blk_delay_queue() to ensure that the request handler
gets reinvoked when it needs to.

Note that in all but one place the old plug re-run delay of
3 msecs is used, even though it probably could be shorter
for performance reasons in some of those cases.

Jens Axboe 401a18e92c block: fix bug with inserting flush requests as sort/merge
With the introduction of the on-stack plugging, we would assume
that any request being inserted was a normal file system request.
As flush/fua requires a special insert mode, this caused problems.

Fix this up by checking for this in flush_plug_list() and use
the appropriate insert mechanism.

Jens Axboe 5e84ea3a9c block: attempt to merge with existing requests on plug flush
One of the disadvantages of on-stack plugging is that we potentially
lose out on merging since all pending IO isn't always visible to
everybody. When we flush the on-stack plugs, right now we don't do
any checks to see if potential merge candidates could be utilized.

Correct this by adding a new insert variant, ELEVATOR_INSERT_SORT_MERGE.
It works just ELEVATOR_INSERT_SORT, but first checks whether we can
merge with an existing request before doing the insertion (if we fail
merging).

This fixes a regression with multiple processes issuing IO that
can be merged.

an accounting bug.

We don't have proper reference counting for this yet, so we run into
cases where the device is pulled and we OOPS on flushing the fs data.
This happens even though the dirty inodes have already been
migrated to the default_backing_dev_info.

Jens Axboe 721a9602e6 block: kill off REQ_UNPLUG
With the plugging now being explicitly controlled by the
submitter, callers need not pass down unplugging hints
to the block layer. If they want to unplug, it's because they
manually plugged on their own - in which case, they should just
unplug at will.

Jens Axboe 73c1010119 block: initial patch for on-stack per-task plugging
This patch adds support for creating a queuing context outside
of the queue itself. This enables us to batch up pieces of IO
before grabbing the block device queue lock and submitting them to
the IO scheduler.

The context is created on the stack of the process and assigned in
the task structure, so that we can auto-unplug it if we hit a schedule
event.

The current queue plugging happens implicitly if IO is submitted to
an empty device, yet callers have to remember to unplug that IO when
they are going to wait for it. This is an ugly API and has caused bugs
in the past. Additionally, it requires hacks in the vm (->sync_page()
callback) to handle that logic. By switching to an explicit plugging
scheme we make the API a lot nicer and can get rid of the ->sync_page()
hack in the vm.

convert it to the (new) real API for delaying queueing a bit. A
default delay of 3 msec is defined, to match the previous
behaviour.

If the iovec is being set up in a way that causes uaddr + PAGE_SIZE
to overflow, we could end up attempting to map a huge number of

Jens Axboe 7ad58c0286 block: fix use-after-free bug in blk throttle code
blk_throtl_exit() frees the throttle data hanging off the queue
in blk_cleanup_queue(), but blk_put_queue() will indirectly
dereference this data when calling blk_sync_queue() which in
turns calls throtl_shutdown_timer_wq().

Fix this by moving the freeing of the throttle data to when
the queue is truly being released, and post the call to
blk_sync_queue().
