An empty flush doesn't have a payload, so it should never be looked at
when considering to possibly requeue a bio for the case when a reshape
is in progress.

In commit 19416123ab ("block: define 'struct bvec_iter' as packed"),
what we need is to save the 4byte padding, and avoid `bio` to spread on
one extra cache line.
It is enough to define it as '__packed __aligned(4)', as '__packed'
alone means byte aligned, and can cause compiler to generate horrible
code on architectures that don't support unaligned access in case that
bvec_iter is embedded in other structures.

The current command UBLK_CMD_DEL_DEV won't return until the device is
released, this way looks more reliable, but makes userspace more
difficult to implement, especially about orders: unmap command
buffer(which holds one ublkc reference), ublkc close,
io_uring_file_unregister, ublkb close.
Add UBLK_CMD_DEL_DEV_ASYNC so that device deletion won't wait release,
then userspace needn't worry about the above order. Actually both loop
and nbd is deleted in this async way.

and ublk_put_device().
Secondly annotate ublk_get_device() & ublk_put_device() as noinline
for trace, especially it is often to trigger device deletion hang

For most of ARCHs, 'nr_cpus=1' is passed for kdump kernel, so
nr_hw_queues for each mapping is supposed to be 1 already.
More importantly, this way may cause trouble for driver, because blk-mq and
driver see different queue mapping since driver should setup hardware
queue setting before calling into allocating blk-mq tagset.
So not overriding nr_hw_queues and nr_maps for kdump kernel.

Commit 4373534a98 ("scsi: core: Move scsi_host_busy() out of host lock
for waking up EH handler") intended to fix a hard lockup issue triggered by
EH. The core idea was to move scsi_host_busy() out of the host lock when
processing individual commands for EH. However, a suggested style change
inadvertently caused scsi_host_busy() to remain under the host lock. Fix
this by calling scsi_host_busy() outside the lock.

Inside scsi_eh_wakeup(), scsi_host_busy() is called & checked with host
lock every time for deciding if error handler kthread needs to be waken up.
This can be too heavy in case of recovery, such as:
- N hardware queues
- queue depth is M for each hardware queue
- each scsi_host_busy() iterates over (N * M) tag/requests
If recovery is triggered in case that all requests are in-flight, each
scsi_eh_wakeup() is strictly serialized, when scsi_eh_wakeup() is called
for the last in-flight request, scsi_host_busy() has been run for (N * M -
1) times, and request has been iterated for (N*M - 1) * (N * M) times.
If both N and M are big enough, hard lockup can be triggered on acquiring
host lock, and it is observed on mpi3mr(128 hw queues, queue depth 8169).
Fix the issue by calling scsi_host_busy() outside the host lock. We don't
need the host lock for getting busy count because host the lock never
covers that.

In blk_mq_mark_tag_wait(), __add_wait_queue() may be re-ordered
with the following blk_mq_get_driver_tag() in case of getting driver
tag failure.
Then in __sbitmap_queue_wake_up(), waitqueue_active() may not observe
the added waiter in blk_mq_mark_tag_wait() and wake up nothing, meantime
blk_mq_mark_tag_wait() can't get driver tag successfully.
This issue can be reproduced by running the following test in loop, and
fio hang can be observed in < 30min when running it on my test VM
in laptop.

blkg_lookup() is called with either queue_lock or rcu read lock, so
use rcu_dereference_check(lockdep_is_held(&q->queue_lock)) for
retrieving 'blkg', which way models the check exactly for covering
queue lock or rcu read lock.

group_cpus_evenly() could be part of storage driver's error handler, such
as nvme driver, when may happen during CPU hotplug, in which storage queue
has to drain its pending IOs because all CPUs associated with the queue
are offline and the queue is becoming inactive. And handling IO needs
error handler to provide forward progress.
Then deadlock is caused:
1) inside CPU hotplug handler, CPU hotplug lock is held, and blk-mq's
handler is waiting for inflight IO
2) error handler is waiting for CPU hotplug lock
3) inflight IO can't be completed in blk-mq's CPU hotplug handler
because error handling can't provide forward progress.
Solve the deadlock by not holding CPU hotplug lock in group_cpus_evenly(),
in which two stage spreads are taken: 1) the 1st stage is over all present
CPUs; 2) the end stage is over all other CPUs.
Turns out the two stage spread just needs consistent 'cpu_present_mask',
and remove the CPU hotplug lock by storing it into one local cache. This
way doesn't change correctness, because all CPUs are still covered.

Request queue quiesce may interrupt flush sequence, and the original request
may have been marked as COMPLETE, but can't get finished because of
queue quiesce.
This way is fine from driver viewpoint, because flush sequence is block
layer concept, and it isn't related with driver.
However, driver(such as dm-rq) can call blk_mq_queue_inflight() to count &
drain inflight requests, then the wait & drain never gets done because
the completed & not-finished flush request is counted as inflight.
Fix this issue by not counting completed flush data request as inflight in
case of quiesce.

The .bd_inode field of block_device is used in IO fast path of
blkdev_write_iter() and blkdev_llseek(), so it is more efficient to keep
it into the 1st cacheline.
.bd_openers is only touched in open()/close(), and .bd_size_lock is only
for updating bdev capacity, which is in slow path too.
So swap .bd_inode layout with .bd_openers & .bd_size_lock to move
.bd_inode into the 1st cache line.

So far, all callers either holds spin lock or rcu read explicitly, and
most of the caller has added WARN_ON_ONCE(!rcu_read_lock_held()) or
lockdep_assert_held(&disk->queue->queue_lock).
Remove WARN_ON_ONCE(!rcu_read_lock_held()) from blkg_lookup() for
killing the false positive warning from blkg_conf_prep().

Monitor work actually introduces one extra context for handling abort, this
way is easy to cause race, and also introduce extra delay when handling
aborting.
Now we start to support cancelable uring_cmd, so use it instead:
1) this cancel callback is either run from the uring cmd submission task
context or called after the io_uring context is exit, so the callback is
run exclusively with ublk_ch_uring_cmd() and __ublk_rq_task_work().
2) the previous patch freezes request queue when calling ublk_abort_queue(),
which is now completely exclusive with ublk_queue_rq() and
ublk_ch_uring_cmd()/__ublk_rq_task_work().
3) in timeout handler, if all IOs are in-flight, then all uring commands
are completed, uring command canceling can't help us to provide forward
progress any more, so call ublk_abort_requests() in timeout handler.
This way simplifies aborting queue, and is helpful for adding new feature,
such as, relax the limit of using single task for handling one queue.

So far aborting queue ends request when the ubq daemon is exiting, and
it can be run concurrently with ublk_queue_rq(), this way is fragile and
we depend on the tricky usage of UBLK_IO_FLAG_ABORTED for avoiding such
race.
Quiesce queue when aborting queue, and the two code paths can be run
completely exclusively, then it becomes easier to add new ublk feature,
such as relaxing single same task limit for each queue.

ublk_cancel_dev() just calls ublk_cancel_queue() to cancel all pending
io commands after ublk request queue is idle. The only protection is just
the read & write of ubq->nr_io_ready and avoid duplicated command cancel,
so add one per-queue lock with cancel flag for providing this protection,
meantime move ublk_cancel_dev() out of ub->mutex.
Then we needn't to call io_uring_cmd_complete_in_task() to cancel
pending command. And the same cancel logic will be re-used for
cancelable uring command.
This patch basically reverts commit ac5902f84b ("ublk: fix AB-BA lockdep warning").

In well-done ublk server implementation, ublk io command won't be
linked into any link chain. Meantime they are always handled in no-wait
style, so basically io cmd is always handled in submitter task context.
However, the server may set IOSQE_ASYNC, or io command is linked to one
chain mistakenly, then we may still run into io-wq context and
ctx->uring_lock isn't held.
So in case of IO_URING_F_UNLOCKED, schedule this command by
io_uring_cmd_complete_in_task to force running it in submitter task. Then
ublk_ch_uring_cmd_local() is guaranteed to run with context uring_lock held,
and we needn't to worry about sync among submission code path any more.

uring_cmd may never complete, such as ublk, in which uring cmd isn't
completed until one new block request is coming from ublk block device.
Add cancelable uring_cmd to provide mechanism to driver for cancelling
pending commands in its own way.
Add API of io_uring_cmd_mark_cancelable() for driver to mark one command as
cancelable, then io_uring will cancel this command in
io_uring_cancel_generic(). ->uring_cmd() callback is reused for canceling
command in driver's way, then driver gets notified with the cancelling
from io_uring.
Add API of io_uring_cmd_get_task() to help driver cancel handler
deal with the canceling.

io_wq_put_and_exit() is called from do_exit(), but all FIXED_FILE requests
in io_wq aren't canceled in io_uring_cancel_generic() called from do_exit().
Meantime io_wq IO code path may share resource with normal iopoll code
path.
So if any HIPRI request is submittd via io_wq, this request may not get resouce
for moving on, given iopoll isn't possible in io_wq_put_and_exit().
The issue can be triggered when terminating 't/io_uring -n4 /dev/nullb0'
with default null_blk parameters.
Fix it by always cancelling all requests in io_wq by adding helper of
io_uring_cancel_wq(), and this way is reasonable because io_wq destroying
follows canceling requests immediately.

Now nvme_ns_chr_uring_cmd_iopoll() has switched to request based io
polling, and the associated NS is guaranteed to be live in case of
io polling, so request is guaranteed to be valid because blk-mq uses
pre-allocated request pool.
Remove the rcu read lock in nvme_ns_chr_uring_cmd_iopoll(), which
isn't needed any more after switching to request based io polling.
Fix "BUG: sleeping function called from invalid context" because
set_page_dirty_lock() from blk_rq_unmap_user() may sleep.

In case of real io scheduler, q->elevator is set, so blk_mq_run_hw_queue()
may just check if scheduler queue has request to dispatch, see
__blk_mq_sched_dispatch_requests(). Then IO hang may be caused because
all passthorugh requests may stay in sw queue.
And any passthrough request should have been inserted to hctx->dispatch
always.

As noted by Michal, the blkg_iostat_set's in the lockless list hold
reference to blkg's to protect against their removal. Those blkg's
hold reference to blkcg. When a cgroup is being destroyed,
cgroup_rstat_flush() is only called at css_release_work_fn() which
is called when the blkcg reference count reaches 0. This circular
dependency will prevent blkcg and some blkgs from being freed after
they are made offline.
It is less a problem if the cgroup to be destroyed also has other
controllers like memory that will call cgroup_rstat_flush() which will
clean up the reference count. If block is the only controller that uses
rstat, these offline blkcg and blkgs may never be freed leaking more
and more memory over time.
To prevent this potential memory leak:
- flush blkcg per-cpu stats list in __blkg_release(), when no new stat
can be added
- add global blkg_stat_lock for covering concurrent parent blkg stat
update
- don't grab bio->bi_blkg reference when adding the stats into blkcg's
per-cpu stat list since all stats are guaranteed to be consumed before
releasing blkg instance, and grabbing blkg reference for stats was the
most fragile part of original patch

This way can simplify userspace for maintaining compatibility because
userspace doesn't need to send command to one device for querying driver
feature set any more. Such as, with the queried feature set, userspace
can choose to use:
- UBLK_CMD_GET_DEV_INFO2 or UBLK_CMD_GET_DEV_INFO,
- UBLK_U_CMD_* or UBLK_CMD_*

Currently copy between io request buffer(pages) and userspace buffer is
done inside ublk_map_io() or ublk_unmap_io(). This way performs very
well in case of pre-allocated userspace io buffer.
For dynamically allocated or external userspace backend io buffer,
UBLK_F_NEED_GET_DATA is added for ublk server to provide buffer by one
extra command communication for WRITE request. For READ, userspace
simply provides buffer, but can't know when the buffer is done[1].
Add UBLK_F_USER_COPY by moving io data copy out of kernel by providing
read()/write() on /dev/ublkcN, and simply let ublk server do the io
data copy. This way makes both side cleaner, the cost is that one extra
syscall for copy io data between request and backend buffer.
With UBLK_F_USER_COPY, it actually becomes possible to run per-io zero
copy now, such as, only do zero copy for big size IO, so it can be
thought as one prep patch for supporting zero copy. Meantime zero copy
still needs to expose read()/write() buffer for some corner case, such
as passthrough IO.

Support pread()/pwrite() on ublk char device for reading/writing request
io buffer, so data copy between io request buffer and userspace buffer
can be moved to ublk server from ublk driver. Then UBLK_F_NEED_GET_DATA
becomes not necessary, so ublk server can allocate buffer without one
extra round uring command communication for userspace to provide buffer.
IO buffer can be located by iocb->ki_pos which encodes buffer offset, io
tag and queue id info, and type of iocb->ki_pos is u64, so it is big
enough for holding reasonable queue depth, nr_queues and max io buffer
size.

Add one reference counter into request pdu data, and hold this reference
in the request's lifetime.
Prepare for supporting to move request data copy into userspace, which
needs to copy request data by read()/write() on /dev/ublkcN, so we have
to guarantee that read()/write() is done on one valid/active request,
and that will be enhanced by holding the io request reference in
read()/write().

task_work_add() is used from early ublk development stage for handling
request in batch. However, since commit 7d4a93176e ("ublk_drv: don't
forward io commands in reserve order"), we can get similar batch
processing with io_uring_cmd_complete_in_task(), and similar performance
data is observed between task_work_add() and
io_uring_cmd_complete_in_task().
Meantime we can kill one fast code path, which is actually seldom used
given it is common to build ublk driver as module.

When handling UBLK_IO_FETCH_REQ, ctx->uring_lock is grabbed first, then
ub->mutex is acquired.
When handling UBLK_CMD_STOP_DEV or UBLK_CMD_DEL_DEV, ub->mutex is
grabbed first, then calling io_uring_cmd_done() for canceling uring
command, in which ctx->uring_lock may be required.
Real deadlock only happens when all the above commands are issued from
same uring context, and in reality different uring contexts are often used
for handing control command and IO command.
Fix the issue by using io_uring_cmd_complete_in_task() to cancel command
in ublk_cancel_dev(ublk_cancel_queue).

Add timeout handler, so that we can provide forward progress guarantee for
unprivileged ublk, which can't be trusted.
One thing is that sync() calls sync_bdevs(wait) for all block devices after
running sync_bdevs(no_wait), and if one device can't move on, the sync() won't
return any more.
Add timeout for unprivileged ublk to avoid such affect for other users which call
sync() syscall.
Meantime clear UBLK_F_USER_RECOVERY_REISSUE for unprivileged ublk since
that feature may cause IO hang too.

So far io_req_complete_post() only covers DEFER_TASKRUN by completing
request via task work when the request is completed from IOWQ.
However, uring command could be completed from any context, and if io
uring is setup with DEFER_TASKRUN, the command is required to be
completed from current context, otherwise wait on IORING_ENTER_GETEVENTS
can't be wakeup, and may hang forever.
The issue can be observed on removing ublk device, but turns out it is
one generic issue for uring command & DEFER_TASKRUN, so solve it in
io_uring core code.

fcloop_fcp_op() could be called from flush request's ->end_io(flush_end_io) in
which the spinlock of fq->mq_flush_lock is grabbed with irq saved/disabled.
So fcloop_fcp_op() can't call spin_unlock_irq(&tfcp_req->reqlock) simply
which enables irq unconditionally.

block size is one very key setting for block layer, and bad block size
could panic kernel easily.
Make sure that block size is set correctly.
Meantime if ublk_validate_params() fails, clear ub->params so that disk
is prevented from being added.

'struct ublk_map_data' is passed to ublk_copy_user_pages()
for copying data between userspace buffer and request pages.
Here what matters is userspace buffer address/len and 'struct request',
so replace ->io field with user buffer address, and rename max_bytes
as len.
Meantime remove 'ubq' field from ublk_map_data, since it isn't used
any more.
Then code becomes more readable.

IO can be started before add_disk() returns, such as reading parititon table,
then the monitor work should work for making forward progress.
So mark device as LIVE before adding disk, meantime change to
DEAD if add_disk() fails.

When investigating one customer report on warning in nvme_setup_discard,
we observed the controller(nvme/tcp) actually exposes
queue_max_discard_segments(req->q) == 1.
Obviously the current code can't handle this situation, since contiguity
merge like normal RW request is taken.
Fix the issue by building range from request sector/nr_sectors directly.

We support mixed merge for requests/bios with different fastfail
settings. When request fails, each time we only handle the portion
with same failfast setting, then bios with failfast can be failed
immediately, and bios without failfast can be retried.
The idea is pretty good, but the current implementation has several
defects:
1) initially RA bio doesn't set failfast, however bio merge code
doesn't consider this point, and just check its failfast setting for
deciding if mixed merge is required. Fix this issue by adding helper
of bio_failfast().
2) when merging bio to request front, if this request is mixed
merged, we have to sync request's faifast setting with 1st bio's
failfast. Fix it by calling blk_update_mixed_merge().
3) when merging bio to request back, if this request is mixed
merged, we have to mark the bio as failfast, because blk_update_request
simply updates request failfast with 1st bio's failfast. Fix
it by calling blk_update_mixed_merge().
Fixes one normal EXT4 READ IO failure issue, because it is observed
that the normal READ IO is merged with RA IO, and the mixed merged
request has different failfast setting with 1st bio's, so finally
the normal READ IO doesn't get retried.

Inside ublk_ctrl_del_dev(), when the device is removed, we wait
until the device number is freed with holding global lock of
ublk_ctl_mutex, this way isn't friendly from user viewpoint:
1) if device is in-use, the current delete command hangs in
ublk_ctrl_del_dev(), and user can't break from the handling
because wait_event() is used
2) global lock is held, so any new device can't be added and
other old devices can't be removed.
Improve the deleting handling by the following way, suggested by
Nadav:
1) wait without holding the global lock
2) replace wait_event() with wait_event_interruptible()

We source root cgroup stats from the system-wide stats, see blkcg_print_stat
and blkcg_rstat_flush, so don't update io state for root cgroup.
Fixes blkg leak issue introduced in commit 3b8cc62987 ("blk-cgroup: Optimize blkcg_rstat_flush()")
which starts to grab blkg's reference when adding iostat_cpu into percpu
blkcg list, but this state won't be consumed by blkcg_rstat_flush() where
the blkg reference is dropped.

Owner of one unprivileged ublk device could be one evil user, which
can grant this disk's privilege to other users deliberately, and
this way could be like making one trap and waiting for other users
to be caught.
So only owner to open unprivileged disk even though the owner
grants disk privilege to other user. This way is reasonable too
given anyone can create ublk disk, and no need other's grant.

unprivileged ublk device is helpful for container use case, such
as: ublk device created in one unprivileged container can be controlled
and accessed by this container only.
Implement this feature by adding flag of UBLK_F_UNPRIVILEGED_DEV, and if
this flag isn't set, any control command has been run from privileged
user. Otherwise, any control command can be sent from any unprivileged
user, but the user has to be permitted to access the ublk char device
to be controlled.
In case of UBLK_F_UNPRIVILEGED_DEV:
1) for command UBLK_CMD_ADD_DEV, it is always allowed, and user needs
to provide owner's uid/gid in this command, so that udev can set correct
ownership for the created ublk device, since the device owner uid/gid
can be queried via command of UBLK_CMD_GET_DEV_INFO.
2) for other control commands, they can only be run successfully if the
current user is allowed to access the specified ublk char device, for
running the permission check, path of the ublk char device has to be
provided by these commands.
Also add one control of command UBLK_CMD_GET_DEV_INFO2 which always
include the char dev path in payload since userspace may not have
knowledge if this device is created in unprivileged mode.
For applying this mechanism, system administrator needs to take
the following policies:
1) chmod 0666 /dev/ublk-control
2) change ownership of ublkcN & ublkbN
- chown owner_uid:owner_gid /dev/ublkcN
- chown owner_uid:owner_gid /dev/ublkbN
Both can be done via one simple udev rule.

Userspace side only knows device ID, but the associated path of ublkc* and
ublkb* could be changed by udev, and that depends on userspace's policy, so
add parameter of UBLK_PARAM_TYPE_DEVT for retrieving major/minor of the
ublkc* and ublkb*, then user may figure out major/minor of the ublk disks
he/she owns. With major/minor, it is easy to find the device node path.

The default queue mapping builder of blk_mq_map_queues doesn't take NUMA
topo into account, so the built mapping is pretty bad, since CPUs
belonging to different NUMA node are assigned to same queue. It is
observed that IOPS drops by ~30% when running two jobs on same hctx
of null_blk from two CPUs belonging to two NUMA nodes compared with
from same NUMA node.
Address the issue by reusing group_cpus_evenly() for building queue mapping
since group_cpus_evenly() does group cpus according to CPU/NUMA locality.
Also performance data becomes more stable with this given correct queue
mapping is applied wrt. numa locality viewpoint, for example, on one two
nodes arm64 machine with 160 cpus, node 0(cpu 0~79), node 1(cpu 80~159):
1) modprobe null_blk nr_devices=1 submit_queues=2
2) run 'fio(t/io_uring -p 0 -n 4 -r 20 /dev/nullb0)', and observe that
IOPS becomes much stable on multiple tests:
- unpatched: IOPS is 2.5M ~ 4.5M
- patched: IOPS is 4.3M ~ 5.0M
Lots of drivers may benefit from the change, such as nvme pci poll,
nvme tcp, ...

Map irq vector into group, which allows to abstract the algorithm for
a generic use case outside of the interrupt core.
Rename irq_build_affinity_masks as group_cpus_evenly, so the API can be
reused for blk-mq to make default queue mapping even though irq vectors
aren't involved.
No functional change, just rename vector as group.

Prepare for abstracting irq_build_affinity_masks() into a public function
for assigning all CPUs evenly into several groups.
Don't pass irq_affinity_desc array to irq_build_affinity_masks, instead
return a cpumask array by storing each assigned group into one element of
the array.
This allows to provide a generic interface for grouping all CPUs evenly
from a NUMA and CPU locality viewpoint, and the cost is one extra allocation
in irq_build_affinity_masks(), which should be fine since it is done via
GFP_KERNEL and irq_build_affinity_masks() is a slow path anyway.

Either ublk_can_use_task_work() is true or not, io commands are
forwarded to ublk server in reverse order, since llist_add() is
always to add one element to the head of the list.
Even though block layer doesn't guarantee request dispatch order,
requests should be sent to hardware in the sequence order generated
from io scheduler, which usually considers the request's LBA, and
order is often important for HDD.
So forward io commands in the sequence made from io scheduler by
aligning task work with current io_uring command's batch handling,
and it has been observed that both can get similar performance data
if IORING_SETUP_COOP_TASKRUN is set from ublk server.

io_uring cmd is supposed to be used in ubq daemon context mainly,
and we should try to avoid to touch it in ublk io submission context,
otherwise this data could become shared between the two contexts,
and performance is hurt.
So link request into one per-queue list, and use same batching policy
of io_uring command, just avoid to touch ucmd in blk-mq io context.

dm-rq implements ->end_io callback for request issued to underlying queue,
and it isn't passthrough request.
Commit ab3e1d3bba ("block: allow end_io based requests in the completion
batch handling") doesn't clear rq->bio and rq->__data_len for request
with ->end_io in blk_mq_end_request_batch(), and this way is actually
dangerous, but so far it is only for nvme passthrough request.
dm-rq needs to clean up remained bios in case of partial completion,
and req->bio is required, then use-after-free is triggered, so the
underlying clone request can't be completed in blk_mq_end_request_batch.
Fix panic by not adding such request into batch list, and the issue
can be triggered simply by exposing nvme pci to dm-mpath simply.

Commit b9684a71fc ("block, loop: support partitions without scanning")
adds GD_SUPPRESS_PART_SCAN for replacing part function of
GENHD_FL_NO_PART. But looks blk_add_partitions() is missed, since
loop doesn't want to add partitions if GENHD_FL_NO_PART was set.
And it causes regression on libblockdev (as called from udisks) which
operates with the LO_FLAGS_PARTSCAN.
Fixes the issue by not adding partitions if GD_SUPPRESS_PART_SCAN is
set.

Without restarting admin queue in __nvme_fc_abort_outstanding_ios(),
it leaves controller not capable of handling admin pt request, and
causes io hang.
Fixes it by restarting admin queue if the caller of __nvme_fc_abort_outstanding_ios
requires to restart queue.
