As preparation for using header records in-pipe mode, replace int fd with struct feat_fd ff in read functions for all header record types. This patch does not change behavior. These guides can be rendered in a number of formats, like HTML and PDF. In order to build the documentation, use ``make htmldocs`` or ``make pdfdocs``.
